Instructions:

Lubricate the penis. Squeeze the sleeve to expel the air.

Anyone who can carefully punch a small hole in the front without pulling on it to allow air to be pushed out easily can do so. This will partially reduce the vacuum, which together with the handcuff holds the penis in place.
